Description: SAINT MARY'S REGIONAL MEDICAL CENTER is at present recruited ( RN ) CASE MANAGER - PER DIEM/VARIED SHIFTS right now, this occupation will be placed in Nevada. Detailed specification about this occupation opportunity kindly read the description below. Required Qualification

1. Two (2) years clinical experience in an acute care setting within the last 5 years.

2. Associate of Arts Degree in Nursing or Diploma Program. BSN preferred.

3. Current licensure as a Registered Nurse in the State of Nevada.

4. Must be able to communicate in English (Speak, Write)

5. Demonstrates: - excellent communication skills both verbal and written - ability to facilitate and collaborate with an int! erdisciplinary team - flexibility to manage multiple complex c! ases simultaneously - attention to detail and follow-up - critical thinking and problem-solving skills.

6. Current BLS (AHA) certificate

7. Case Management Certification (CCM) preferred.

8. Ability to work weekends.

Job description/Responsibilities
Responsible for the quality and resource management of all patients that are admitted to the facility from the point of their admission and across the continuum of the health care management. Works on behalf of the advocate, promoting cost containment and demonstrates leadership to integrate the health care providers to achieve a perceived seamless delivery of care. The methodology is designed to facilitate and insure the achievement of quality, clinical and cost effective outcomes and to perform a holistic and comprehensive concurrent review of the medical record for the medical necessity, intensity of service and severity of illness.
