Lead House Supervisor/ Infection Control Coordinator RN

Location: Elko Nevada

Description: Northeastern Nevada Regional Hospital is currently interviewing Lead House Supervisor/ Infection Control Coordinator RN right now, this position will be placed in Nevada. Further informations about this position opportunity please read the description below. Lead House Supervisor oversees:
1. Provides for adequate staffing of nursing personnel for all shifts! through the development, preparation, and maintenance of staffing schedules that meet hospital needs and apply to variable census conditions. Is aware of skill level for each shift to ensure quality of care. Ensures staffing is consistent with approved budgetary constraints.
2. In the absence of the CNO and appropriate Nursing Managers, coordinates all hospital activities relative to routine and/or emergency care.
3. Receives report for floors and provides report to appropriate personnel at the end of the shift. Provides the appropriate Manager with a report of activities that occurred throughout the shift. Makes rounds throughout the hospital.
4. Responsible for patient room assignments and ensuring there are appropriate doctor's orders. Notes orders as appropriate.
5. Carries the keys to all departments. Knows location of supplies and all emergency equipment and who to call if an item cannot be found. Responds to departments' calls for equipment/supplies.!
6. Responsible for all non-routine medication delivery to! the floors.
7. Relief nurse for each unit as appropriate. Responsible to all codes.
8. Calls in surgery crews, med flights, initiates disaster tree and coordinates until appropriate staff arrive. Responsible for calling coroner, police, childcare services, etc.
9. Completes CQI forms for all departments for Quality Control - e.g. incident reports, crash cart checks, refrigerator checks, etc. Provides for concurrent medical record review to determine patient needs and appropriateness of documentation.
10. Provides for education of nursing personnel - e.g. mock codes, assistance with techniques, etc.

Case Management Supervisor oversees:
1. Maintains established departmental policies and procedures, objectives, Continuous Quality Improvement Program, Safety, Environmental and Infection Control Standards.
2. Collaborates with the director and department members to develop department goals and objectives and to establish and implement pol! icies and procedures for department operation.
3. Consults with the Director and department members on matters pertaining to Continuous Quality Improvement, Risk Management, Safety, Infection Control and Care Coordination.
4. Participates in providing educational programs and in-service meetings.
5. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops.
6. Screens admissions for medical necessity.
7. Performs utilization certification activities.
8. Monitors medical orders for appropriateness and facilitates timely completion.
9. Monitors diagnostic test results to facilitate adequate intervention and documentation.
10. Facilitate sequencing of patients clinical services for logical order and timely response.
11. Ensures appropriate use of health care resources across the continuum by the patient population.
12. Conducts patient screening to id! entify patients appropriate for care coordination.
13. Completes a ! comprehensive health assessment of the patients' physical, psychological, social, environmental, financial and functional status through patient interviews and using existing data in the patient's medical records.
14. Assists medical and nursing staff to identify specific patient care needs.

15. Assesses family, institutional and community support systems and resources.
16. Identifies new resources if gaps exist in the service continuum.
17. Links the patient with the most appropriate institutional or community resources.
18. Coordinates service providers responsible for furnishing services needed by a given patient and all payment sources that may reimburse for those services.
19. Advocates and negotiates on behalf of the patient for scarce resources.
20. Provides appropriate education for the patient, the family and community support systems regarding the value of self-care by the client and honoring client choices.
21. Assists patient! s to negotiate health care system.
22. Monitors the patients' progress toward goal achievement and periodic reassessment of changes in health status.
23. Monitors patient patterns regarding decisions and choices and their impact on health care utilization and patient outcomes (e.g. readmissions, frequent ER visits).
24. Initials, coordinates and facilitates patient care conferences when indicated.
25. Participates in hospital committee and staff meetings to provide information and education on pertinent care coordination/performance improvement issues.
26. Monitors and reports on risk management and performance improvement indicators.
27. Becomes involved in interagency groups and community agencies, as appropriate, to support and represent the care coordination progress.
28. Participates in public speaking, marketing and hospital-based education programs.

II. GENERAL HOSPITAL STANDARDS
1. Reports to work and from breaks as scheduled! and on time, prepared to assume duties
2. Works the hours schedule! d and do not work overtime or lunch breaks without the Director's approval.
3. Badges in and out appropriately.
4. Follows policy for reporting in sick or absent 100% of the time.
5. Schedules all paid time off with the Director's approval and gives at least two weeks' notice.
6. Reports to work in proper attire.
7. Adheres to hospital and departmental policy and procedures 100% of the time.
8. Applies principles of safety in work practices by demonstrating good body mechanics. Uses proper lifting techniques.
9. Reports on the job accidents/injuries immediately to supervisor.
10. Follows procedure for equipment use and maintenance.
11. Demonstrates ability to use appropriate channels of communication to address concerns, transmit information, and solve problems by following chain of command.
12. Limits personal phone calls.
13. Provides hospital with a copy of current license/certification as needed.
14. Enhances professional ! growth and development through participation in educational programs, in-service classes, seminars, and affiliation with professional organizations.
NV RN license. BLS

3-5 years experience in general nursing or community health nursing preferred; some management experience preferred. Basic knowledge about health care systems; social services and community agencies.
