Detailed specification about this position opportunity please give attention to these descriptions. POSITION SUMMARY (includes, but is not limited to):
The Case Manager will work in conjunction with nursing, physicians, ancillar! y hospital departments, patients and families to identify and coordinate post-hospital needs of all patients, infant through geriatrics, in order to promote quality, continuity of care and safety for the patient. The Case Manager is responsible for carrying out physician orders regarding transfers, home health care, durable medical equipment and the specific discharge needs of the patient, placing patients in the most appropriate setting upon discharge. He/she will monitor utilization of resources, track any avoidable days identified, respond to insurance requests for clinical reviews in a timely manner utilizing Interqual criteria and follow review protocols for the department.
Qualifications
Education:
Graduate of a nursing program - diploma, associate degree or baccalaureate program (BSN Preferred)
License:
Current Nevada R.N. License
Certification:
Current Basic Cardiac Life Support required within 3 m! onths of hire. Case management certification preferred
Minimum of 1year case management experience in an acute care facility or 2 years minimum experience in a managed care environment or rehab facility.
Special Skills:
Must have strong communications skills both written and verbal, computer skills and knowledge of Interqual criteria.
