Position Duties:
- Provide for cost-effective, quality, and best-practice outcomes through collaboration and communication with the client, the family, and multi-disciplinary teams.
- Assesses the client/family to identify their strengths and barriers relating to in psychosocial, financial, medical history, functional status, goals, treatment plan and evolving needs
- Develops a plan of care through interdisciplinary collaboration, which identifies realistic short- and long-term goals to achieve highest level of independence. Modifies the plan of care with statuses changes.
- Oversees, modifies, and Monitors services being provided for continued appropriateness and modifies as needed.
- Provides education to the client and family regarding the injury or disease process, treatment options, and care plan.
- Effectively documents outcomes encompassing clinical, functional, financial, and Customer satisfaction areas
- Identifies and discloses all situations related to Ethics which might result in exposure to legal liability of poor quality care negatively affecting the client
- Provides testimony in collaboration with organization, clients, and their legal counsels as needed.
- Performs other related duties as assigned in accordance with the necessary job function.
Requirements:
- CA RN license
- Valid driver’s license and automobile insurance
- 3 to 5 Years of Acute Care Experience
- 1 year of Complex Case Management with progressive responsibilities in Home/Outpatient Settings
- Specialized focus in Trauma, Complex Diseases, and Disability resulting in permanent changes to highest level of function and significant loss in levels of independence.
- BSN preferred.
- CCM preferred.
- Experience in rural or underserved populations preferred.
