Our road warriors will conduct structured and thorough in home evaluations with f! ocus on CKD, DM and PAD. All our NP’s will receive the necessary training and supplies to supplement your existing clinical expertise and judgment and enable you to complete screenings and submit necessary documentation.
Responsibilities:
Perform pre-visit confirmation call.
Provide friendly, safe, comfortable experience for member while performing Chronic Conditions Screening and exam in member’s home.
Review and interpret/verify members’ medical and social history.
Determine and validate members Primary Care Physician.
Performs appropriate medical screening (Vitals, Blood Draw, Urine Sample, ABI, Monofilament) procedures based on Educerus Health protocol.
Submit completed documentation within 24 hours of exam.
Complete exam addendum upon review & receipt of patient lab result! s. Lab data is reviewed and addendum is completed within Educe! rus Health’s patient portal. Addendum will reflect clinical assessment of conditions per program guidelines.
In home screenings are conducted Monday â€" Saturday with assessments taking 60 to 70 minutes.

Valid accreditation from the AANP or ANCC
Participates in all mandatory compliance and training programs.
Current Nurse Practitioner’s license in state of practice.
Compliant with OSHA, CLIA, State Board of Nursing and Federal DEA Board of regulations
Current CPR certification
Minimum 1 year clinical experience as a Board Certified Nurse Practitioner. NEW GRADS are WELCOME to apply
Demonstrated experience with venipuncture â€" lab specimen procurement is an integral component of our work
Experience completing AB! I exam
Proven ability to independently perform comprehensive adult/geriatric physical exam
Demonstrated ability to identify, diagnose and document chronic disease in adult/geriatric
Demonstrated ability to recognize conditions, characteristics and behaviors that place adult/geriatrics at risk
Previous experience with proper documentation and charting practices to ensure a full and accurate reflection of patients’ health status
Effective verbal communication skills. (Spanish speaking a plus)
Pleasant interpersonal demeanor and ability to work with Medicare population
Committed to excellence, best practices, and superior customer service with every client
Must have valid state driver license and automobile insurance.
Must be willing to travel in 50-mile radius.
Must be willing to drive in various weather conditions.
Must be able to lift/move equipment bag/kit (50lbs)
