Centra Healthcare Solutions is seeking an experienced Registered Nurse (RN) to work as the Director of Case Manager that is licensed! or in the immediate process, and qualifies, of obtaining RN licensure in this respective state to work in the specialty area of Case Management.
The Director of Case Management is responsible for the assessment, planning, coordination, development and evaluation of the Case Management program. This includes the functions of case management, utilization management, discharge planning and social services. Accountable for the assessment and monitoring of the care and services rendered by the department staff. Also responsible for staffing levels, competency and educational needs of the case management staff to ensure effectiveness of the coordination and control of resources. Provides consultation and education to facility leadership, clinical and financial personnel and medical staff members related to case management, utilization and discharge planning practices. Ensures compliance with local, state, federal and private agencies as relates to Case Management and Utiliza! tion Management. Provides leadership in the fulfillment of UHS! /VHS driven initiatives and expectations.
Requirements include:
Must have graduated from an accredited school.
Current state license as a Registered Nurse (RN) or willing to obtain licensure in another state, assuming that all pre-requisites have been satisfied.
Current CPR certification.
Current BLS card.
Minimum of two years experience in leadership/management position.
5-7 years experience in a Utilization Review- Director role in an acute care hospital setting preferred.
Must have knowledge of social and physical factors that affect functional status at discharge, and knowledge of community resources to meet post discharge clinical and social needs.
Duties and Responsibilities
Assesses patient needs, signs and symptoms indicating physiologic and psychosocial changes in the patient's condition
Utilizes the nursing process to assess, plan, implement and evaluate patient care
Collects, analyzes, and interprets dat! a and information from health care members and documents actual and/or potential nursing diagnoses.
Documents the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
Provides a safe, comfortable and therapeutic patient environment.
Initiates and evaluates patient and/or family education.
Revises and documents the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions.
Monitors the safety and effectiveness of the environment and equipment.
