The Sr RN -Telemonitoring is primarily responsible for oversight of patients monitored remotely via in home telemonitoring devices. This position reviews vital signs and pati! ent responses to selected clinical surveys to assure the plan of care is adjusted to prevent hospitalizations and unplanned emergency care and to meet the patient's home care needs via coordination with the attending physician, other members of the clinical team, and the patient/caregivers. The Sr. RN will utilize the company's clinical information system extensively in the performance of key duties. The Sr Telemonitoring RN works primarily in the office but may be required to visit the patients in their home from time to time.
Responsibilities:
Provides guidance and teaching to clincial staff
Utilizes company's educations tools to assist with efficent management of patients to prevent hospitalizations
Coordinates with DME provider for the delivery and the set up of the telemonitors
Utilizes company's outcome benchmarking tools to analyze clinical outcome data on a concurrent and retrospective basis
Assumes individual resoponsibility for ! professional competence and professional nurse practices

Requirements:
Nursing diploma or equivalent education
2+ years of experience as a RN - hospital, acute, cardiology
Knowledge of chronic disease - COPD, diabetes, heart failure
Current and acitve unrestricted RN license in the state of NV
Strong critical thinking skills
Stong assessment skills
Participate in on-call weekend rotation (about once a month)
Able to plan, organize, prioritize and direct the work of others and communicate objectives clearly.
High level critical thinking skills to analyze multiple sources of information to formulate logical conclusions and actions.
Excellent verbal and written communication skills; high level of attention to detail and demonstrated competency in core leadership skills.
Basic PC skills to include Windows, Outlook or other email system and experience working with a clinical application system.
! Completion of a Telemonitoring Certification within a timeframe decided upon by management
Completion of a national certification in an area related to chronic disease management within a timeframe decided upon by management
