The RN Case Manager/Health Coach for the health management program assists members with chronic conditions (i.e Diabetes, COPD, CAD, Heart Failure, Hyp! ertension and Asthma) to manage their health and decrease disease exacerbations leading to increased utilization of health care resources, such as emergency room and hospital stays. The RN assesses medical indicators and health management information. The RN teaches the members about self management techniques as related to their chronic illness.
Primary responsibilities:
1. Assess, teach and collaborate with the member identified with chronic health conditions to carry out their provider's treatment plans and improve health outcomes.
2. Document health management assessments and interventions.
3. Develop plans of care that meet the member and provider's goals.
4. Make appropriate internal and external referrals to assist the member with medication compliance, health needs and psychosocial needs.
5. Review health outcomes and gather member information for statistical information and reporting.
6. Manage a case! load ensuring monitoring of each member's health status at le! ast every 60 days for members at high risk.

Requirements
Requirements:
Current, unrestricted RN license in the State of Nevada required.
2+ years clinical nursing experience required.(preferably in Med/Surg, acute care, direct care experience or experience as a Telephonic Case Manager/Disease Manager/Health Coach).
Basic level of experience with Microsoft Office applications, with the ability to navigate a Windows environment required. Excel highly preferred.
Preferred Assets:
Diabetes experience or Diabetes Educator highly preferred.
Experience with Chronic Disease i.e. COPD, CHF, Hypertension, Asthma, CAD.
Health Coaching, Health Education,and patient teaching experience.
Knowledge of Hedis measures.
A background in managed care.
! Telephonic Case Management or Disease Management experience.
Certified Case Manager (CCM).
Strong organizational skills and multitasking abilities will be keys to success.
