Tour of Duty: 7:30 A.M. to 4:00 P.M., Monday - Friday.

Background and-or Security Investigation required DUTIES:
The VA Southern Nevada Healthcare System is currently recruiting for Social Workers to serve as the primary contact with the local Public Hou! sing Agencies (PHA) and to provide case management-based services (dire! ctly or through referral) necessary to ensure that Veterans care is coordinated with the VA medical center, community health, and agencies that serve the homeless population. The incumbent also represents the VA at community service provider meetings and ensure that the representation is positive and appropriate to the position held. Duties include, but are not limited to the following functions:

a . Clinical - The incumbent conducts psychosocial assessments as well as assessments of persons in their environments and develops treatment plans and identifies goals of treatment based on these assessments and in collaboration with the Veteran and family, where possible, and with the interdisciplinary treatment team. Incumbent possesses the skills to offer psychosocial interventions and measures outcomes to interventions.

(1) Develops and researches community resources that are available to provide contunuity of care and enhance the quality of life for the Vete! rans.

(2) Attends all social work staff meetings, team meetings and arranges other duties to ensure attendance at these required meetings to obtain needed organizational communications for improving performance and knowledge regarding the organization.

(3) Participates on Social Work and hospital committees as requested.

(4) Provides or assists in providing back-up coverage for co-workers during their absence to ensure that patient care is provided in a timely manner.

(5) Evaluates the veterans enviromental situation, including capabilites, and arrives at a sound conclusion regarding the veterans vulnerability and the need for prioritization of admission to the HUD/VAHS program.

(6) Interviews the Veterans and their family members or significant others to establish facts about the Veteran's situation, presenting problems and their causes, and the impact of such problems on the Veterans functioning and health.

(7) Provide! s referral, rehabilitation, and program admission activities that inclu! de referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-VA entitlement programs. The incumbent will demonstrate general knowledge of Veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension. Facilitates referrals based upon Veterans needs and eligibility.

(8) Consistently communicates and treats customers (Veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Incumbent is able to provide the Veterans with consistent information according to established policies and procedures. Handles conflict and problems in dealing with the Veteran constructively and appropriately.

(9) Advises and informs the supervisory social worker of significant developments, problems, and progress in the assigned programs.

(10) ! Must have a safe driving record and possess a valid driver's license.

b. Administration

(1) Participates fully in peer review, assessing the quality of professional Social Work practice standards provided in the organization.

(2) Responsible for identification of patient problems requiring consultation with other multi-disciplinary medical center staff and making appropriate referrals.

(3) Social workers ensure that appropriate referrals to specialty programs, including community resources, are accomplished. This may include ordering the following consults: Mental Health Programs, (i.e., substance abuse, trauma, and vocational rehabilitation); Long Term Care and Community Care Programs (i.e., nursing home referrals, Homemaker/Home Health Aide, In-Home Respite, Community Residential Care and Adult Day Care).

(4) Being fully accountable for practice, including entering patient visits, measuring outcomes to interventions, and meetin! g productivity standards.

c. Research :
(1) Partici! pates in research as appropriate through case findings and referral of patients to clinical investigators engaged in research or through participating in research studies.

(2) Applies new approaches to patient care through integration of approved investigative and research methods in clinical practice, basing interventions on evidence of effectiveness.

d. Education

(1) Provides and, where appropriate, referring patients, families and significant others for counseling in health maintenance and disease prevention.

(2) Promotes patient independence and achievement of optimal health through appropriate psychosocial education and community referrals.

(3) Serves as clinical field instructors for social work graduate students enrolled in CSWE-accredited education programs as part of the VHA Associated Health Trainee Program.

(4) Participates fully in Social Work educational development through staff and section meetings.
(5) Assumes responsibility for professional development by attending relevant training and regularly reviewing the literature to improve clinical competencies. QUALIFICATIONS REQUIRED:

Basic Requirements:

  • U.S. citizenship.
  • Master's degree in social work from a program approved by the Council on Social Work Educate (CSWE), including but not limited to the Masters in Social Work (MSW) degree.
  • Must be proficient in spoken and written English
  • For the GS-11 level, requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level OR in addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting.
  • The licensure or certificati! on requirements for persons who are otherwise qualified, pending comple! tion of state prerequisites for licensure/certification examinations may be waived.

Applicants must demonstrate experience that equipped the applicant with particular Knowledge, Skills, and Abilities (KSA) to perform successfully the duties of the position and this is typically in or related to the work of the position to be filled. To be creditable, specialized experience at the GS-11 level, must be equivalent to at least one year at the next lower grade (GS-09) level.

  • Knowledge of community resources, how to make appropriate referrals to community and othe! r governmental agencies for services, and ability to coordinate services.
  • Ability to independently assess the psychosocial functioning and needs to patients and their family members to formulate and implement a treatment plan, identify the patient's problems, strengths, weaknesses, coping skills, and assistance needed, in collaboration with the patient, family, and interdisciplinary treatment team.
  • Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological and the differential influences of the environment, society, and culture.
  • Knowledge and experience in the use of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries,! common medications, and their effect/side effects, and medical termino! logy.
  • Knowledge of psychosocial treatment and ability to independently implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. This requires independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques.
  • Ability to independently provide counseling and/or physiotherapy service to individuals, groups and families. social workers must practice within the bounds of their license or certification. For example, some states may require social workers providing psychotherapy to have a clinical level of licensure.
  • Ability to provide consultation service s to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new social workers and social work graduate stu! dents. Ability to serve as a field instructor for social work graduate students who are completing VHA field placements.
  • Ability to independently evaluate his/her own practice through participation in professional peer review case conferences, research studies, or other organized means.
  • Knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking.
