St. Rose Dominican Hospital, Siena Campus, 4 East Medical Surgical unit provides services to a patient population that include adult and geriatric patients. Predominant service lines include care of the stroke and psychiatric patient, post operative oral-pharyngeal, abdominal, and orthopedic surgeries, and variety of medical diagnoses. There is a strong focus on quality and evidence-based nursing practice. Safe Patient Handling Equipment located on each unit or available to each to unit to assist staff to safely and appropriately mobilize patients including repositioning in bed, ambulating, lateral transferring or transferring to chair/commode.

Job Summary:
The Patient Care Coordinator (PCC) is a Registered Nurse responsible for coordinating healthcare services across the continuum of care by monitoring, evaluating and modifying the treatment plan to achieve optimal patient outcomes

under the direction of the Director's of Case Management and Medical Surgical Services. The PCC employs critical thinking to integrate objective data with knowledge gained from an initial interview and ongoing assessment including the objective and subjective experiences and health care history of the patients. The PCC will use this critical thinking process to apply the best, evidence based practice and research data to interdisciplinary care coordination through admission to discharge planning. The PCC will continually evaluate quality and effectiveness of Coordinated Patient Care practice and seek to optimize outcomes for an appropriate and safe discharge for patients. Medical/clinical knowledge, technical and clinical skills, clinical judgment, interpersonal skills and communication as indicated in the current Comprehensive Accreditation Manual for Hospitals are essential components of nursing and case management practice. The PCC performs within the scope and standards of practice set forth by the American Nurses Association and any professional organizations specific to specialty. Additional Professional Organizations may be referenced when appropriate to guide practice and standards within the work place.

This position is represented by CNA and is covered by the terms and conditions of the applicable collective bargaining agreement.

Experience

Minimum - Three (3) years acute hospital clinical experience as a Registered Nurse. One year (1) of health care related Case Management experience and/or One year (1) or greater current Charge Nurse or Nursing Management experience.

Preferred - Two years (2) current Acute Hospital Charge Nurse or Nursing Management Experience. Two Years (2) of current hospital Case Management experience or Case Management experience in related field of health care. Certification in related clinical specialty area. Validation of active participation and /or leadership in a unit or facility based committee.

Education

Minimum - Graduate of an accredited school of nursing Bachelor of Science in Nursing (Current Case Managers and Charge Nurses that are selected will be given 3 years to complete BSN).

Training

Assaultive Behavior Course within 30 days of hire.

National Institute of Health Stroke Scale certification within 30 days of hire. *

Knowledge of all aspects of documentation in Midas, CERME, CIS, SMS, and the AWARIX system supporting patient flow and continued care management. * Basic understanding of desk top computers and work related software * Demonstrates proficiency in nursing process, problem solving and the performance improvement process * Maintains current knowledge in basic health care economics, trends, and reimbursement methodologies and applies knowledge to daily activities.

Special Skills

Demonstrates organizational, collaboration, negotiation skill and leadership ability. Strong customer service skills and the ability to handle highly confidential

information. Must have a working knowledge of Nevada Board of Nursing rules and regulations. Required to flexible in scheduling i.e., weekends, holiday and floating. Demonstrates the ability to understand the unique infant, child, adolescent, adult and geriatric population and effectively meet those needs relative to growth, assessment and development of the patients. Demonstrates the ability to understand and instruct employees on the unique care of the patient populations in their clinical service area. * Monitors the clinical care of patients and provides clinical support to improve patient care, staff satisfaction, patient satisfaction and patient outcomes * Coordinates clinical resources to achieve Clinical Quality Outcomes: Including but not limited to Core Measures, NPSGs, Stroke Measures of Success, SCIP.

Licensure

Required: RN=Current license as a Registered Nurse in the State of Nevada; CPRBLS = Basic Life Support Certificate

National Institute of Health Stroke Scale certification within 30 days of hire.

Preferred: Specialty certifications as applicable; ACLS = Advanced Life Support Certificate.
